Role Description

The Registered Nurse (RN) in this Clinical Coordinator role will be responsible for providing in-home patient care (including infusions, medications, etc.), managing care plans, conducting QA visits with caregivers, charting and managing patient records, conducting new client assessments, participating in community education events and engaging caregiver & medical staff, patients and their families. Patients are all located in the Sarasota/Bradenton area, schedule is 5 days per week, allowing for the work/life balance that has found to be short supply in healthcare.

Qualifications Needed

  • Valid Registered Nurse (RN) license in the state of Florida
  • 2 years of experience as a Registered Nurse in a private duty home care/hospice environment
  • Ability to work independently with excellent communication, time management, clinical assessment and critical thinking skills
  • Knowledge of and experience with care management and basic infusion therapy and protocols
  • Experience with electronic medical records (EMRs) and care management systems
  • Ability to work collaboratively with medical staff and patients
  • Valid driver's license and insurance
  • BSN (preferred)
