Care Team Associate

Nextaff is searching for a Patient Care Coordinator with strong medical administrative and patient communication skills to serve as a part-time (20-24 hours weekly) Care Team Associate for our Primary Care client in Sarasota, FL. In this temporary assignment, you will be responsible for providing critical administrative support to patients in an important program, as well as internal team members, on a daily basis by conducting inbound & outbound calls to/from patients, documenting all interactions in EMR, scheduling patient appointments and follow-up, answering patient questions, attending internal meetings and processing all documentation in a timely and accurate manner. You will be the key link between patients and internal departments, providing excellent customer service and maintaining key documentation.
