Position/ Type:            Occupational Therapist, Permanent, Full time

 

Description:    We are looking for an enthusiastic and motivated Occupational Therapist to join our team!  We are a skilled nursing facility with short-term & long-term care consisting of 102 beds providing great opportunities for growth.  The facility has large gym with windows and a full team in place with all disciplines.  We promote independence and encouragement through a robust support network. We utilize electronic documentation via Net Health. Our services include comprehensive post-acute care, discharge planning program, long-term care services, incontinence, Alzheimer's and dementia care programs. We value creativity and compassion here, so we hope you will bring unique ideas to our team that enhance the care we provide to our clients.  We are looking for someone who loves what they do and is looking for a challenge. Rate:                            Minimum $38 per hour  (rate negotiable)

 

Hours per week:          Monday to Friday, 35 hours per week, 8:00AM to 4:30PM (no weekend rotation), no travel required

 

Qualifications:

    Graduate of an accredited Occupational Therapy (OT) program

    Current state licensure

    Valid Driver’s license

    Excellent verbal, written and interpersonal communication skills

    Results-oriented with strong critical thinking and problem-solving skills

Responsibilities:

    Address client’s aspects of performance to support engagement in occupations that affect health, well-being and life quality

    Select and adapt the appropriate per case occupational therapy treatment model, method and approach to direct the process of interventions

    Apply interventions to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL)

    Teach clients new ways of approaching tasks and educate them on how to break down activities into achievable components

    Observe, note, and report on progress of long and short term treatment goals

    Collect data and document processes followed

    Regularly follow-up client
