Description: We are looking for an enthusiastic and motivated Occupational Therapist to join our team! We are a skilled nursing facility with short-term & long-term care consisting of 102 beds providing great opportunities for growth. The facility has large gym with windows and a full team in place with all disciplines. We promote independence and encouragement through a robust support network. We utilize electronic documentation via Net Health. Our services include comprehensive post-acute care, discharge planning program, long-term care services, incontinence, Alzheimer's and dementia care programs. Rate: Minimum $38 per hour (rate negotiable)
Hours per week: Monday to Friday, 35 hours per week, 8:00AM to 4:30PM (no weekend rotation), no travel required
Qualifications:
● Graduate of an accredited Occupational Therapy (OT) program
● Current state licensure
● Valid Driver’s license
● Excellent verbal, written and interpersonal communication skills
● Results-oriented with strong critical thinking and problem-solving skills
Responsibilities:
● Address client’s aspects of performance to support engagement in occupations that affect health, well-being and life quality
● Select and adapt the appropriate per case occupational therapy treatment model, method and approach to direct the process of interventions
● Apply interventions to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL)
● Teach clients new ways of approaching tasks and educate them on how to break down activities into achievable components
● Observe, note, and report on progress of long and short term treatment goals
● Collect data and document processes followed
● Regularly follow-up client
